About our forms: Our forms are created using Adobe Acrobat, and require the free Acrobat Reader.
At the bottom of each form you will have 2 options:
- Option - SAVE THE DOCUMENT
- This option will let you SAVE the document to your computer to complete or print at a later time.
- Option - PRINT THE DOCUMENT
- This option will let you PRINT the document and fill it out by hand.
All documents must have a signature, therefore all documents must be printed and faxed.
To download the document and complete later, just use the right mouse button to click on Request for Proposal. Select "Save Link As" (in Netscape) or "Save Target As" (in Internet Explorer). You will then get the "Save As" Dialog box. Select the directory in which you want to save the file, then click on "Save". Be sure to remember the directory you save the file to, so you can open the file after you download.
